How to create an application form?

Hi all... I would like to know how can I create an application form in my network. I need that people to fill up the form and when they click "ok" or something similar, this form will be sended to me... or via e-mail or via personal message...

Is that possible?

Cant see why not! Put it into an iframe and it´s donne just the way you want!

Thanks... I did the form in DOCS and got the iframe but where can I post it? Should I create a widget or someting? If yes, how can I do that?

U so wrong! Just a matter of create the form and use the form code to embed it on a page, do that and its donne, see this example
