Help please with forums and groups

Hello,

I am really struggling with forums and groups. And I wonder if any one can help put me right. I have set up and deleted so many groups, group categories and forums I just cannot understand how they work and how i can present them in a way that users can see clearly what's a forum and what's a group etc... 

Here is my URL, but it does not look like what I'm trying to acheive. www.lediabete.ning.com

Ideally, I would create groups and forums in a hierarchical way :

I want 3 main groups :
People,
Topics,
Location

Then in the People group there'd be subgroups :
Parents
Family members 
Teens
Kids
Young Adults
Adults
Teachers and Colleagues
Medical Staff
Researchers
Recently diagnosed

Topics
T1D and similiar forms
T2D and similiar forms
Exercise, Fitness and sport
Diet, Food and Nutrition
School and Uni
Work
Complications
Concomittant illnesses
D burnout

Location
Brussels
Flanders
Wallonia
Switzerland
France
Paris
Canada
DOM TOM

In each subgroup, people will create forums or discussions and they will add tags or categories to help make things clear. I have started a landing page but there is a bug in the slideshow. The landing page willl be my home page. 

Thanks in advance if anyone can shed some light before I pull all my hair out.

    Here is how I see it - you can create 3 group categories here - http://take.ms/HulH3

    There will be People, Topics and Location. Then you can create groups - "Family members ", "Young Adults", "T2D and similar forms", "Brussels" etc, and assign each group to one of the categories (People, Topics, Location). So basically the main categories will be displayed here - http://take.ms/SrQsz and your members will be able to select one of the main categories to check group in that list. And of course, in each group, you will be able to post discussions, have comment walls, blogs, etc.
